Recreation Department Consent Policy
This policy outlines the consent and responsibilities for participation in all programs, activities, and events organized by the Avon Lake Recreation Department. By registering for a program, participants agree to the terms and conditions listed below.
 
  1. Consent to Participate:
    By registering for any program, activity, or event, participants (or their legal guardians if under 18) confirm their understanding of the program’s nature and voluntarily consent to participate. They also confirm that they are physically capable of engaging in the activities.
 
  1. Liability Waiver and Release:
    Participants agree to release, indemnify, and hold harmless the City of Avon Lake, its employees, agents, contractors, and volunteers from any claims or liabilities arising from injury, illness, or incidents occurring during participation. This includes, but is not limited to, accidents, physical injuries, or property damage.
 
  1. Emergency Medical Consent:
    In case of an emergency, participants grant permission for the City of Avon Lake to provide or arrange for emergency medical treatment, including first aid, medical care, and transportation to a medical facility. Participants understand they are responsible for any associated costs.
 
  1. Photo, Video, and Media Consent:
    By registering for a program, participants grant the City of Avon Lake permission to capture and use photos, videos, or audio recordings of themselves (or their child) for promotional purposes. These materials may be used across various media formats, including print, digital, social media, and broadcast, without compensation.
 
  1. Code of Conduct and Behavior Expectations:
    Participants agree to adhere to all program rules and regulations and respect the rights of fellow participants and staff. The Avon Lake Recreation Department reserves the right to remove any participant who engages in disruptive or unsafe behavior that negatively impacts the experience of others.
 
  1. Special Accommodations:
    Participants with disabilities or special needs requiring accommodations should contact the Recreation Department prior to registration. The Department will work to provide reasonable modifications to ensure equal access and inclusion in all programs.
 
  1. Acknowledgment of Risks:
    Participants acknowledge that some programs involve physical activity and understand the inherent risks, including the possibility of severe injury, permanent disability, or death. Participants are encouraged to consult with a physician before participating if they have any health concerns.
 
  1. Program Modifications and Cancellations:
    The Avon Lake Recreation Department reserves the right to modify or cancel programs due to insufficient enrollment, weather, or unforeseen circumstances. If a program is canceled, the Department will notify participants and provide a refund or alternative options when possible.
 
  1. Fees & Registration Information:
    All participants must be fully registered and have paid all fees before attending any program. By registering, participants certify that all information provided is accurate. Providing false information may result in ineligibility for participation in Recreation Department programs.
